I hereby give permission to Healthwise Kliniek to perform chiropractic treatments. I understand that treatments are aimed at improving spinal function and relieving pain and may include manual therapy, muscle relaxation techniques, and other related therapies.

Risks and Benefits

I have been informed about the possible risks and benefits of chiropractic treatments. I understand that although rare, certain risks such as bruising, muscle stiffness, temporary increase in pain, or rare more serious complications may occur.

Consent and Questions

I have had the opportunity to ask questions about my treatment and these have been answered satisfactorily. I understand that I can ask questions or withdraw my consent at any time.

The current version of the privacy policy available on the website is the only version that applies as long as you visit the website, until a new version replaces the current version.

Any request for this must be accompanied by a copy of a valid proof of identity on which you have signed and stating the address at which you can be contacted. You will receive an answer to your request within 1 month after the submitted request. Depending on the complexity of the requests and the number of requests, this period can be extended by 2 months if necessary.
